Post-Hospital & Post-Rehab Care Program
This service is tailored for older adults recovering from hospitalization or rehabilitation. Recognizing that prolonged hospital stays can lead to diminished physical and cognitive abilities, the program aims to:
- Reduce unnecessary readmissions to acute care facilities
- Alleviate the financial and emotional burdens on family caregivers
- Educate patients and caregivers on symptom management
- Promote aging in place by decreasing the level of required care
The care plan is collaboratively designed by a registered nurse and therapist, with services executed by Life Enhancement Caregivers. This program operates in conjunction with Medicare services to support chronic illness management and overall wellness.
